The poppy plant, Papaver somniferum, is the source for non-synthetic narcotics. It was grown in the Mediterranean region as early as 5000 B.C., and has since been cultivated in a number of countries throughout the world. The milky fluid that seeps from incisions in the unripe seed pod of this poppy has, since ancient times, been scraped by hand and air-dried to produce what is known as opium. A more modern method of harvesting is by the industrial poppy straw process of extracting alkaloids from the mature dried plant. The extract may be in liquid, solid, or powder form, although most poppy straw concentrate available commercially is a fine brownish powder. More than 500 tons of opium or equivalents in poppy straw concentrate are legally imported into the United States annually for legitimate medical use.
In 2009, current cigarette smoking among persons aged 12 or older was lower in the West (20.3 percent) and Northeast (21.9 percent) than in the South (24.5 percent) and Midwest (25.7 percent). Use of smokeless tobacco was also higher in the Midwest and South (4.0 and 4.1 percent, respectively) than in the West (2.8 percent), which in turn was higher than in the Northeast (2.1 percent).
Although the rate of past month illicit drug use was higher among unemployed persons compared with those from other employment groups, most drug users in 2009 were employed. Of the estimated 19.3 million current illicit drug users aged 18 or older in 2009, 12.9 million (66.6 percent) were employed either full or part time. The number of unemployed illicit drug users increased from 1.3 million in 2007 to 1.8 million in 2008 and 2.5 million in 2009, primarily because of an overall increase in the number of unemployed persons between 2007 and 2009. Increases also were seen in the numbers of past month illicit drug users among adults who were employed part time (from 3.1 to 3.6 million) and those in the "other" employment category (retired persons, disabled persons, homemakers, students, and other persons not in the labor force) (from 3.1 to 4.0 million). NSDUH estimates the numbers of persons who were employed full time were 122.2 million in 2008 and 114.8 million in 2009; the estimated numbers of unemployed persons were 9.0 million in 2008 and 14.7 million in 2009. The rate of current illicit drug use among unemployed adults was 19.6 in 2008 and 17.0 percent in 2009; although this difference is not statistically significant, the rate in 2009 may reflect the inclusion of persons who had been employed in 2008 but were unemployed at the time they were interviewed in 2009.
Marijuana was the most commonly used illicit drug. In 2009, there were 16.7 million past month users. Among persons aged 12 or older, the rate of past month marijuana use and the number of users in 2009 (6.6 percent or 16.7 million) were higher than in 2008 (6.1 percent or 15.2 million) and in 2007 (5.8 percent or 14.4 million).

Mental health and substance abuse treatment consists of patients being persuaded, motivated, or involuntarily committed to addiction treatment. The main goal is to help stabilize the acute symptoms of the psychiatric illness and/or the drug or substance abuse disorder. Another important goal of mental health and substance abuse treatment is to motivate patients to continue treatment once the acute addiction crisis is stabilized or the involuntary commitment expires. Dealing with ambivalence regarding recovery, working through denial of one's drug addiction and mental illnesses or both, and becoming motivated for continued care are other important goals. This initial part of treatment usually takes several weeks, but for some patients it takes longer to become engaged in recovery and to stabilize from the acute effects of dual disorders.

The next step of mental health and substance abuse treatment is early drug addiction recovery. This involves learning to cope with desires to use chemicals, alcohol, cocaine, meth, or other drugs. It also teaches patients to avoid or cope with people, places, and things that represent high-risk addiction relapse factors. Mental health and substance abuse treatment roughly involves the first 3 months following stabilization. However, some patients take much longer in this phase because they do not comply with addiction rehab treatment, continue to abuse drugs, experience exacerbations of psychiatric symptoms, or experience serious psychosocial problems or crises.
