Children playing, eating and sleeping in a meth lab are surrounded by the deadly gases. All adults and children removed from a meth lab are showered, decontaminated, and dressed in haz/mat suits before they are removed from the area. Children living at, or near a meth lab are exposed to immediate dangers and to the on going effects of chemical contamination. The extent of the lasting or long-term physical effects on meth users or their children who are exposed to a meth lab are not known
Students who use drugs are statistically more likely than nonusers to drop out of school, bring guns and knives to school, and be involved in physical attacks, property destruction, stealing, and cutting classes (SAMHSA, 2004). Drug Situation: Sharing a border with Canada, Washington is a transshipment point for drugs and monies entering Canada, as well as a Canadian pseudoephedrine, Canadian marijuana (BC Bud), and other drugs entering the United States. Washington has severe clandestine methamphetamine laboratory problems. Mexican National poly-drug organizations dominate the drug trade and many have added MDMA as a drug they can now supply. The greater Yakima Valley is home to large scale poly-drug trafficking organizations responsible for the distribution of drugs throughout the United States.

Cocaine: Cocaine and crack cocaine are readily available. Crack cocaine is largely limited to inner city areas. Cocaine is consumed in the state, and smuggled into Canada for redistribution, where it commands a higher price than in the United States. According to a recent Royal Canadian Mounted Police report, up to 24 tons of cocaine enter Canada each year. BC Bud marijuana from British Columbia, Canada is sold or traded in the United States for cocaine to be sold in Canada.

Heroin: Mexican black tar and brown heroin are the primary types of heroin found throughout Washington. Mexican poly-drug trafficking organizations remain the primary source of black tar heroin in Washington. Heroin is smuggled from Mexico to Washington by a variety of methods, including vehicles equipped with concealed compartments.

Methamphetamine: Methamphetamine is one of the most widely abused controlled substances in Washington. Two "varieties" are generally encountered; Mexican methamphetamine, which is either manufactured locally or obtained from sources in Mexico, California, or other Southwest Border States, and methamphetamine which is produced locally by area violators. Of the two types, Mexican methamphetamine continues to flood the market. Methamphetamine is available in multi-kilogram amounts throughout the state. Canadian pseudoephedrine, utilized in the manufacture of methamphetamine, is frequently seized at clandestine laboratory sites. Washington State continues to see an influx of crystal “ice” methamphetamine.

Club Drugs: MDMA (Ecstasy) is smuggled into the state from elsewhere in the country, Canada, and Europe. Club drugs are growing in popularity across the state, and have been targeted in several successful investigations by the DEA. Smuggling methods have included MDMA shipped in express mail packages, concealed in lumber, hidden compartments in vehicles, and transported in luggage on international or domestic flights. Mexican National poly-drug traffickers have become more involved in MDMA trade in Washington.

Marijuana: Marijuana is readily available in multi-pound quantities throughout the state. Three types are normally encountered: locally grown (either from indoor or outdoor grow operations), Canadian BC Bud from British Columbia, and Mexican marijuana. Of these varieties, locally grown sinsemilla and BC Bud are preferred, because they have a far superior THC content than Mexican grown marijuana. Canadian BC Bud is the most prevalent variety in many areas, given the proximity to the border and the extent of cross border smuggling. BC Bud marijuana is sold or traded in the United States for cocaine to be sold in Canada. The arid and warm mountainous areas of Eastern Washington are being utilized by Mexican National organizations to grow multi-thousand plants of marijuana. These large scale marijuana gardens have been found on National Forest land and Native American Tribal lands.

Other Drugs: The primary method of pharmaceutical drug diversion continues to be forgery and telephone prescriptions by non-registrants. Illegal dispensing and prescribing by practitioners still exists in the state. OxyContin is often the target of criminals involved in burglary and armed robbery. Sting operations in Washington State have found high school students selling Oxycontin, cocaine, and marijuana. Often these drugs were purchased over the internet.

DEA Mobile Enforcement Teams: This cooperative program with state and local law enforcement counterparts was conceived in 1995 in response to the overwhelming problem of drug-related violent crime in towns and cities across the nation. There have been 409 deployments completed resulting in 16,763 arrests of violent drug criminals as of February 2004. There have been 12 MET deployments in the State of Washington since the inception of the program: Puyallup, Everett, Chehalis, Thurston/Yelm, Seattle, Lakewood, Lynnwood, Vancouver, Bremerton, Pierce County, Auburn, and Snohomish.

DEA Regional Enforcement Teams: This program was designed to augment existing DEA division resources by targeting drug organizations operating in the United States where there is a lack of sufficient local drug law enforcement. This Program was conceived in 1999 in response to the threat posed by drug trafficking organizations that have established networks of cells to conduct drug trafficking operations in smaller, non-traditional trafficking locations in the United States. Nationwide, there have been 22 deployments completed resulting in 608 arrests of drug trafficking criminals as of February 2004. There has been one RET deployment in the State of Washington since the inception of the program, in Seattle.

The Kulshan Creek ARIS program will provide comprehensive intervention services to youth residing in the Kulshan Creek neighborhood of Mount Vernon, Washington. Kulshan Creek is a densely populated area with a 95% Latino population who have been traditionally under-served by local social service agencies. The primary program focus will be to provide intervention services to young neighborhood Latinos aged12 through17 years. An early intervention/prevention component will serve Kulshan Creek children from preschool age to 11 years old. Additionally, project efforts and services will integrally involve neighborhood families through parent education, family assessment and resource linking. Goals of the Project are: Increase the availability of culturally-based activities to Kulshan Creek youth that encourage positive physical, emotional, psychological and social development; develop and maintain a broad community coalition comprised of youth, family members, residents, businesses, law enforcement and youth-serving professionals to guide and support juvenile violence prevention efforts in the Kulshan Creek Neighborhood; prevent violence and reduce delinquency among Kulshan Creek neighborhood youth; and increase access to early intervention and rehabilitative services for Latino children and youth while offering parenting resources and other support to their families.

The Youth with Co-occurring Disorders (YCD) Violence Prevention Project will target Latino youth with co-occurring mental health and substance abuse disorders involved in the juvenile justice system in Yakima County, Washington. The Office of Juvenile Justice and Delinquency Prevention and the National GAINS Center for People with Co-occurring Disorders in the Justice System report that two-thirds of youth involved in the juvenile justice system have co- occurring disorders. The goals and objectives of the YCD Violence Prevention Project are to: Implement an integrated mental health and substance abuse treatment program for Latino YCD involved in the juvenile justice system that is effective in improving pro-social behaviors and preventing violence and substance abuse. The Committee will: I) screen, determine eligibility, assess, and recruit YCD; 2) provide Multisystemic Therapy (MST) for YCD; and 3) recruit, retain, and train additional staff. The process and outcomes of the collaboration, its activities, and the treatment program will be evaluated and the evaluation results will be disseminated. The Committee will develop a Quality Improvement Task Force, review evaluation results, consult with experts about program adaptations, and develop and monitor quality improvements. The Committee will: I) develop resources from public and private sources; and 2) advocate for policy change that promotes effective treatment programs.

Yakima County in Washington State will implement a collaborative Nurse-Family Partnership program utilizing the Olds' model of nurse home visiting. An infant mental health speciality component will be added. The purpose of the program is to decrease risk factors by modifying behaviors of mothers targeting 5 primary domains: personal health, environmental health, maternal role development, maternal life course, and friend and family support. Outcomes for improved birth outcomes, decreased child abuse and neglect, decreased child abuse and neglect, decreased dependence on public economic support, and decreased mental health problems of the mother ande infant are expected.

The Yakima County Jail Diversion Program will serve chronically and acutely mentally ill adults ages 18 and older who come into contact with the Yakima County or City of Yakima law enforcement, court or jail systems. The program will utilize a multi-point jail diversion model to avoid or dramatically reduce the length of incarceration for acutely and chronically mentally ill adults. The model will divert individuals from jail at three contact points. The first diversion point will be the initial law enforcement contact (pre-booking). Chronically or acutely mentally ill adults (including those with co-occurring disorders) may be diverted prior to booking to a voluntary plan and voluntary mental health services. The second diversion point will be at arraignment (post-booking). The target population who have been arrested and who are identified as potential candidates through a mental health assessment would be considered for diversion. The third diversion point will be at adjudication (post-booking/post adjudication). Acutely or chronically mentally ill individuals with complex mental health or co-occurring disorders and who have need of more complex or long-term interventions are targeted at this diversion point. In order to implement the diversion model outlined above, Yakima County will re-direct and expand its existing jail mental health/substance abuse services to a diversion focus. Yakima County will partner with Central Washington Comprehensive Mental Health.

The Puyallup Tribal Health Authority (PTHA) Child and Adolescent SIG project will develop the infrastructure for full implementation of a wraparound service delivery model, which is culturally appropriate and cost effective for American Indian/Alaska Native (AI/AN) youth that are struggling with substance abuse and mental health problems, such as serious emotional disturbances, in the Pierce County service delivery area. The PTHA will use the grant funds to build the infrastructure to promote, support, and sustain the mental health and substance abuse treatment intervention capabilities for the target population.

The project will implement best practice alternatives to seclusion and restraint in Washington State. This will be accomplished through a multifaceted program of training and changes in policies and practices at all three of the state's public inpatient psychiatric hospitals. Western State Hospital and Eastern State Hospital care for adults, older adults and forensic populations; the Child Study and Treatment Center cares for youth under age 18, including forensic and non-forensic patients. The goal of this project is to implement best practice alternatives in 100% of the state hospital wards and units, resulting in the complete elimination of seclusion and restraint episodes. Hospital leadership attendance will participate in workshops offered by the National Technical Assistance Centers, followed by diffusion of training for all staff throughout the facilities. Training includes development of facility- specific plans that address the variety of interventions needed to reduce seclusion and restraint, including interventions addressed to leadership and organizational change, effective use of data, workforce development, use of seclusion/restraint reduction tools, consumer empowerment, and debriefing techniques. A process and outcome evaluation will be conducted by an experienced research institute in close consultation with the national evaluation center at HSRI and with the hospitals' advisory boards to evaluate the effects of the project and to assess process strengths and weaknesses and make ongoing improvements in project operations.

This project will continue the State's effort to build infrastructure to collect data and report the remaining Mental Health Block Grant Uniform Reporting System Developmental Measures. Grant efforts will focus on (1) local provider training to improve data quality, (2) implementation of web-based technology using DS2K + data standards to collect, report, and improve accessibility of data, and (3) strengthening internal and external database linkages. Project outcomes will include consistent data definitions, timely capture of data, improved measure of service outcomes and client change, improved data quality, and enhanced ability to analyze and report on developmental measures such as school attendance, school performance, and involvement with the criminal justice system. The project outcomes will be evaluated based on the ability to produce the data required for URS and other desired reporting. The project will also be evaluated in terms of its ability to produce data that is useful to and is used by system stakeholders.

The Harborview Center for Sexual Assault and Traumatic Stress is a University of Washington hospital specialty program providing services to children and adults affected by sexual assault and violence. The Child Traumatic Stress program will increase capacity at the Center, offer an enhanced program of Parent Child Interaction Therapy, improve mechanisms for identifying and linking children served within the medical center, create a collaboration with community providers serving ethnic minorities to refine culturally specific methods of identifying and treating children, develop and evaluate a components based treatment training program, collaborate with the Medical Examiner's Office to link children whose parents have died violently to services, and manage a website resource for practitioners across the state.

Partnership for Anti Violence Enhancements (PAVE) A Project of the High Point Youth Service Provider Coalition. The Partnership for Anti-Violence Enhancement (PAVE) project will serve low-income youth living in the High Point Garden Community, a public low-income housing project located in Southwest Seattle. There will be a special emphasis on Southeast Asian and East African youth ages 13-19 who come from immigrant and refugee families. The High Point Youth Service Provider Coalition via the Partnerships for Anti-Violence Enhancements (PAVE) project has the following major goal: To increase positive opportunities and protective factors for High Point youth and negate community and individual risk factors, emphasizing: enhancement of systems for early and ongoing identification of youth at- risk and development of a community support safety net; opportunities for increased youth bonding to community and adults; community recognition for youth; healthy beliefs and clear standards; education/academic commitment; and behavioral and social skill building.

Asian Counseling and Referral Services (ACRS), once of the Pacific Northwest's leading refugee and immigrant serving community-based organizations proposes the Eastside Asian Pacific Islander Young Women's Project whose goal is to expand and enhance the existing Seattle based Teen Dating Violence collaboration to represent a greater geographical region. Through collation building, grantee plans to gain commitment to implement a local community-based approach towards the prevention of relationship violence amongst young Asian Pacific Islander (API) women. The project seeks to accomplish this goal through the development of a coalition of community and agency- based stakeholders and by planning and delivering protective factor building interventions to API young women and their peers to prevent the victimization of API teen girls and to strengthen the overall positive development of youth in their community. According to a recent study from the Journal of the American Medical Association, approximately 1 in 5 female public high school students reported being physically and/or sexually abused by a dating partner and that the relatively high levels of dating violence occurred in all ethnic and racial groups. Additionally the report notes that girls who experience dating violence are also more likely to become pregnant, smoke heavily, abuse alcohol and other substances, use laxatives and vomiting for weight loss- and they are 8 to 9 times more likely to have attempted suicide (Silverman, 2001). Neighborhood House proposes Project HANDLE (HIV/AIDS) Network Development and Life-skills Experience) to prevent and reduce HIV and substance abuse (SA) among high-risk Cambodian refugee community in Seattle/King County (KC). HANDLE activities include culturally appropriate peer education and intensive services to high-risk youth including tutoring, prevention classes and job skills. Washington State has the third largest number of Cambodian refugees (15,00) in the US with most living in Seattle/KC. The Cambodian American youth of Seattle/KC are at very high risk for HIV/SA because of the favorable attitudes toward drug use, fighting and other and-social behaviors, the HIV/AIDS epidemic in Cambodia is and sexual promiscuity. Local needs assessment data show that compared to American youth, Cambodian American youth are nearly three times more likely to have sexual intercourse and 93% of the Cambodian-American youth do not talk to their parents about HIV/STIs infection, sex or reproductive health problems. The incidence of reported AIDS cases in 2001 among Asians in Seattle is double the national rate for Asian/Pacific Islanders (Seattle/KC Public Health Department, 2001). Of the 37 HIV non-AIDS cases in Seattle King County, 19% are Cambodian refugees. The goals of HANDLE are to: 1) reduce high-risk behaviors and increase protective behaviors for high-risk Cambodian youth and their families; 2) increase parent's bicultural parenting skills and knowledge of HIV/SA; 3) increase community knowledge and awareness about HIV/SA; and 4) create Cambodian network and resources for HIV/SA. In collaboration with Cambodian Women's Association and other partners, our activities include intensive services to 50 high risk Cambodian youth and their parents including HIV/SA education classes, year-round youth employment and tutoring, parenting classes, community volunteering and youth cultural activities.

Strategic Prevention Framework State Incentive Grants (SPF SIG)-Washington The Strategic Prevention Framework State Incentive Grants are used to advance community-based programs for substance abuse prevention, mental health promotion, and mental illness prevention. The SPF SIG implements a five-step process known to promote youth development, reduce risk-taking behaviors, build on assets, and prevent problem behaviors. The five steps are: (1) conduct needs assessments; (2) build state and local capacity; (3) develop a comprehensive strategic plan; (4) implement evidence-based prevention policies, programs and practices; and (5) monitor and evaluate program effectiveness, sustaining what has worked well. These grants will allow the programs to provide leadership, technical support and monitoring to ensure that participating communities are successful. The success of the grants will be measured by specific measurable outcomes, among them: abstinence from drug use and alcohol abuse, reduction in substance abuse-related crime, attainment of employment or enrollment in school, increased stability in family and living conditions, increased access to services, and increased social connectedness. Washington State's Strategic Prevention Framework will provide the resources necessary to enhance existing assessment processes, implement and evaluate evidence-based strategies to address critical needs based on epidemiological data, and establish reporting procedures that track progress toward preventing substance abuse and related problems, including mental illness, delinquency, and violence.
